Care Coordination Study - Maui

Are you a cancer patient?  We need your help to improve cancer care!

We would like to invite you to participate in our study!

In this study you will:

• Watch educational videos about cancer, care coordination, & self advocacy
• Videos can be viewed at home on the tablet device provided at no cost
• Complete questionnaires at the
  • start of the study and
  • after completing your adjuvant chemotherapy
• It will take about 15 minutes to complete

ELIGIBILITY:

• 18 years or older
• Have newly diagnosed early stage cancer (stage I, II, or III)
• Planned to start or within 1 month of starting adjuvant chemotherapy treatment
• Reside on Maui
